Job Description and Duties

The Health Record Technician I will be directed by the Health Record Technician II (Supervisor) in the Health Information Management Department. The incumbent will be responsible for the following duties but not limited to:
• The ability to physically travel to/from and perform duties on all treatment units in the secured area of the hospital.
• Perform on-unit documentation review, chart maintenance, thinning and/ or filing
• Regularly walk long distances
• Routinely carry/ transport medical record binder (estimated at 10-15 pounds)
• Conduct accurate, structured, concurrent, quantitative audits within the secured area of the facility
• Assign ICD-10 codes in accordance with ICD-10-CM guidelines. Abstract data from a variety of sources
• When assigned, attend and participate in appointments for Patient Access to Medical Record
• Analyze medical records for deficiencies when a patient is discharged or goes to court
• Communicate incomplete record findings to responsible staff and take steps to have the record completed

MINIMUM QUALIFICATIONS: Education Requirements: Equivalent to completion of the twelfth grade. (Additional qualifying experience may be substituted for the required education on a year-for-year basis.) Either I One year of California state service clerical experience which includes training and supervised work experience in the procurement and processing of health data. Or II Two years of clerical experience in the medical or health records section of a health facility. (Successful completion of an academic curriculum in medical records science in an accredited school may be substituted for the required general experience on a year-for-year basis.)

Salary shown is from the State of California Civil Service Pay Scale and may be subject to a reduction due to collective bargaining.  Additional information regarding appointment salary is available upon receipt of a contingent job offer.
